The transportation policies for both the Hamilton Wentworth District School Board and the Hamilton Wentworth Catholic District School Board state that a Parent/Guardian
must accompany all Junior Kindergarten and Kindergarten students from the school bus.
If you wish to designate another person to accompany your child (ren) please fill out the form below indicating the person(s)
designated. It is recommended that the authorized person be a responsible adult or responsible young person over the age of twelve (12).
Student(s) unaccompanied will be returned to the school. Any changes and/or deletions can be completed through
this website.
This authorization is effective for this school year only.
